The information will be collected in identifiable format by Homewood Health on behalf of the Treasury Board Secretariat (TBST B S) and is necessary for the proper administration of the ISCSI S C S program, which supports the objectives of the OPSO P S People Plan, Mental Health and Well-being Action Plan, and the OPSO P S Leadership Pledge.

All information will be de-identified, aggregated, analyzed and managed by Homewood Health in a manner consistent with the Data Standards for the Identification and Monitoring of Systemic Racismopens in a new tab and the OPSO P S Human Resources Privacy and Security Guidelinesopens in a new tab. To ensure the confidentiality of respondents, there will be no reporting on any group where there are less than ten respondents.

Homewood Health will provide de-identified and aggregated reporting to designated representatives within the Centre for Employee Health, Safety and Wellness (CEHSWC E H S W), Treasury Board Secretariat (TBST B S). CEHSWC E H S W which will use the de-identified and aggregate information to gain insights on client experiences with the program for the purpose of evaluating the program to ensure it continues to meet the needs of OPSO P S employees and their families and to inform any future improvement to services.

TBST B S does not collect identifying information such as your name, email address or IP address.
